Add app shell, engine host, UX rules and glass design

- Four placeholder screens (home, setup, interview, report) switched by a small app state; focus moves to each screen's heading
- Setup starts with Video interview (default) or Phone screen; picking it mounts the voice engine once at app level, and it stays mounted through the interview; the video engine is lazy-loaded
- COOP/COEP headers on the dev and preview servers
- docs/UX.md: rules for downloads, mic, privacy, device check, interview screen, report, look and feel, accessibility
- Calm dark glass design with one accent colour and a radius scale; solid fallback for no backdrop-filter or reduced transparency
- Report shows only measured numbers and quotes: scores removed from the plan, STAR parts out unless the review model passes the evals
- Remove the Vite starter files
