Harden the agent with four fixes from the failed Dagster run

  1. A step that starts with cd <dir> no longer fails when the shell is already inside <dir>. The planner also stops adding cd lines.
  2. A recovery patch that keeps less than 60 percent of the original command is rejected as drift, so a fix cannot swap the docs flow.
  3. The summary names the step that gave up, not a step recovered earlier.
  4. A producer killed by SIGPIPE (yes | tool) no longer fails the step. 205 offline tests and 38 live tests pass.
