Advance subagent scans in the same state update as the snapshot

Turn end recorded the snapshot (clearing the pending files it wrote) and the running subagents' scan positions in two separate state updates. If the second failed, the next scan re-read the snapshotted edits and re-baselined them from content that could include user edits. SaveStep now records the scan positions itself, on a written step and on a skipped one alike.
