Cover StopFailure end to end and tidy its tests

  • Integration test: a prompt followed by stop-failure leaves the session IDLE with the turn's step saved. Without the StopFailure mapping it stays ACTIVE.
  • The transcript flush sentinel matches stop-failure as a prefix of the stop command; document that and pin it with a test so tightening the match doesn't silently slow API-error turns.
  • Fold the two missing-hook drift tests into one table test built from a real install, and simplify the agent-guide TurnEnd cell.
