Show more files with capped per-file matches in code search output

Terminal output now favors breadth: up to 10 files with the first 3 matches each and a dim "+ N matches" overflow line, plus a blank line before the first result. Text display fetches a deeper page (100) so file variety can fill, unless --limit is passed; JSON is unchanged.
