Carry a turn's prompt only from an offset that still marks its start

The turn-start offset into prompt.txt was trusted whenever it fell inside the file. If a condensation cleared the file mid-turn and later prompts refilled it past that length, the offset landed mid-prompt and a fragment was carried. The offset is now used only on a boundary (start, end, or a separator); otherwise the whole file is this turn's.
