docs: describe the natsmsg/jsconsumer/backoff subpackages

Root doc.go and README now cover the consumer-side layers and the dependency posture: the root package stays nats.go-only; natsmsg and jsconsumer add the OpenTelemetry API, all three subpackages use nats.go/jetstream, and importing only the root links none of that.
