Rename module to github.com/entireio/go-nuts

Rename the project from entwine to go-nuts. The module path becomes github.com/entireio/go-nuts; the Go package identifier becomes nuts (a valid identifier — the module path's hyphen is not), so call sites read nuts.Connect(...). Log/error prefixes, package docs, README, and test cert CNs updated to match.
